Membership Withdrawal Form

What are you withdrawing from?

For Private Lessons

  1. Membership may be withdrawn by submitting the online withdrawal form. The form must be received by the 15th of the month to take effect at the end of that month.
  2. You may withdraw the request at any time before it takes effect, but your teacher and/or time slot cannot be guaranteed.
  3. Active membership is mandatory to participate in our events, exams, and recitals. All registrations will be removed from the roster at the time of membership withdrawal.
  4. All make-up lesson credits on your account expire in 30 days from the withdrawal effective date.
  5. Membership is not transferable.
  6. Opus 1 Music Studio does not provide tuition refunds under any circumstances.
  7. Opus 1 Music Studio reserves the right to terminate membership, in which case prorated tuition will be refunded.

Example: if you submit the online cancellation form on May 20, you will still be billed for June and there will be no bill after June.

15 days notice are required for Private Lesson withdrawals requests. Requests submitted by the 15th take effect at the end of the current month; requests submitted on or after the 16th take effect the following month. Group class withdrawals follow the separate group class policy and are reviewed by the front desk.
